The intent is to increase use of the hospital’s MyChart personal medical record system to communicate with doctor offices, set appointments, refill prescriptions and more effectively manage patient health.

Module – MyChart
Working collaboratively with MetroHealth, Connect Your Community created a module to teach MyChart personal medical record system to MetroHealth patients. Cleveland Housing Network and Ashbury Senior Computer Community Center are teaching the MetroHealth patients how to use MyChart.
